[origin: EP0803501A1] Substituted guanidine derivatives of formula (1) <CHEM> wherein each of R1 to R4 and Z is an organic group, m is 0, 1 or 2 and Y1, Y2, Y3 and Y4 form, together with the carbon atoms to which they are attached, a saturated or unsaturated carbocyclic or heterocyclic ring; and pharmaceutically acceptable salts thereof; are inhibitors of the sodium/proton (Na<+>/H<+>) exchange transport system and can therefore be used in the treatment or prophylaxis of, for example, hypertension, arrhythmia, angina pectoris, cardiac hypertrophy, diabetes mellitus, organ disorders associated with ischemia or ischemic reperfusion, cerebroischemic disorders, diseases caused by excessive cell proliferation, and diseases caused by endothelia cell injury.
