[origin: WO0066009A1] This invention is methods, and devices for forming a conduit (18) to place a target vessel in fluid communication with a source of blood, such as the aorta or a heart chamber. The conduit (18) may include a first conduit (118) including synthetic vascular graft material, such as PTFE, coupled to a second conduit (110) in the form of a hollow tissue structure, such as an autologous vessel. The first conduit (118) is secured to the target vessel. The second conduit (110) is secured to the first conduit (118), and is placed in fluid communication with a blood source.
