[origin: WO2010066507A1] According to the invention, vital data are continuously recorded and/or measured for the remote diagnostic monitoring and support of patients. Said data are interpreted by means of signaling technology and evaluated with regard to the progression thereof and the context in which said data were recorded/measured. A link is made from the vital data to a patient profile and evaluated based on threshold values in order to recognize deviations in the condition of a patient from a previously determined target condition. A categorization is done as to whether an interaction with the patient is required or not required in a specified time frame based on the evaluation.
