[origin: WO2014162100A1] The invention relates to a remote screening and monitoring method for chronic diseases, comprising steps of recording, for each subscribed patient, information representative of his/her health status, and steps of collecting additional data in order to monitor the health of the patient. The data recording step consists in sending a first request to the patient's terminal, requesting the input of general information, and subsequently processing the response to the request on the server, consisting in recording the information in the space of the base assigned to the patient and generating a new complementary request formulated according to the analysis of the general information, for the collection of specific information, said specific information then being processed, consisting in recording same in the space of the base assigned to the patient and optionally formulating a new request for the collection of additional specific information, and stopping the recording step when the processing operation provides information relating to the integrity of the recorded patient information.
