[origin: WO2018091806A1] The invention concerns a system for collecting the informed consent of a patient, comprising a server having a memory for recording a plurality of multimedia digital files and programming means for controlling the operation of the system, in such a way as to: establish a computer session between computer equipment of the patient, equipped with a browser, and said server, to control the transmission, to said computer equipment of the patient, of multimedia digital files to record at least one piece of information concerning the opening of a session between the computer equipment of the patient and the server, as well as information transmitted by said computer equipment of the patient by means of a digital form. The system further comprises a plurality of hardware media each having an identical information area, an area for signature by the patient, an area for signature by the practitioner and a pair formed by a unique username and password corresponding to the digital information recorded beforehand in a memory of said server, in order to activate the step of opening a secure session between the server and computer equipment, and in that said programming means for controlling the operation of the system comprises means for recording, in a memory space associated with the unique username used for opening the secure session, time-stamped information relating to the processing of the multimedia digital files on the computer equipment corresponding to said secure session.
