Abstract (en)

The invention relates to medicine, in particular to medical equipment, namely, to instruments and methods for performing resections of joint tissues, and it can be used when carrying out arthroscopic surgeries on joints, for instance, knee ones.The method for radiofrequency resection of meniscus is carried out using a system of arthroscopic instruments, while arthroscopic ports are installed in a joint, into one of those an arthroscopic instrument is inserted, areas of meniscus damage are identified, after that the damaged area of the meniscus is removed and wounds are sutured. The removal of the damaged area of the meniscus is carried out with the use of an arthroscopic instrument for radiofrequency resection of the meniscus, which is inserted into the joint in the closed position of an outer tube of the instrument, after that it is transferred to its open position, the meniscus resection is continued, leaving one dissected fragment of the meniscus, and a loop electrode located in an inner tube of the instrument is rotated around its own axes using a proximal manipulator, after resection of the meniscus, the outer tube is moved to its closed position using a distal manipulator and removed from the joint, then the dissected fragment of the meniscus is evacuated using an arthroscopic clamp.In the first variant, an arthroscopic instrument, connected to an energizer and a drainage system, comprises a handle with a "start" button thereon, an inner tube with openings, namely, an aspiration opening and an opening in which a loop electrode is arranged, while the inner tube is moved out of a distal end of the handle, and a drainage tube and a power cable are connected to the proximal end, wherein the inner tube has two openings, one of which contains the loop electrode, and the other is an aspiration one, which is directed to a drainage channel.In the second variant, the arthroscopic instrument comprises a proximal manipulator of axial rotation of the inner tube, which manipulator being disposed on the handle proximally to the "start" button thereof.In the third variant, the arthroscopic instrument comprises a distal manipulator of linear motion disposed on the handle distally to the "start" button thereof, and the inner tube is disposed inside an outer tube provided with a beveled tip.Increasing the accuracy of resection, improving visualization during surgery, reducing the level of trauma to the tissues of the joint and reducing the time of surgery.
