    Description

    TECHNICAL FIELD



    [0001] The present invention mainly relates to an organ fastening tool used for gastrostomy (PEG: percutaneous endoscopic gastrostomy).

    BACKGROUND ART



    [0002] In previous gastrostomy, two puncture needles were punctured from the body surface to the stomach wall, a suture was inserted from one of the puncture needles, and the suture was hooked by the loop body which was inserted from the other needle and captured, thereby forming the loop. Stomach is lifted by pulling the looped suture, and the body wall and the stomach wall are fixed by further tying the sutures together with each other. Thereafter, the large through hole is formed in the body wall and the stomach wall by deeply puncturing the guidewire and dilator from the body surface, and the stomach fistula is established by indwelling the indwelling catheter there.

    [0003] In a general gastrostomy, it is necessary to fix the body wall and the stomach wall is in at least three positions (possibly four positions) so as to surround the gastrostomy scheduled portion. After the stomach fistula is established, the sutures are cut and removed.

    [0004] This method is a highly invasive because it is necessary to puncture the two puncture needles in respective fixing positions, and in some cases it is difficult to capture the suture by the loop body in the stomach due to the skill of the doctor.

    [0005] On the other hand, it became possible to be low invasive and easy procedure according to the instrument of Patent Document 1 (prior art), since the stomach wall can be fixed to the body wall by only one puncture needle, as shown in FIG. 11, and the like. According to this procedure, it became unnecessary to loop the suture since the procedure can be performed with a single puncture needle by inserting T-shaped suture instrument in which the suture is engaged with the center of the rod-shaped engaging portion rod and lifting the stomach wall, instead of tying the two sutures.

    [0006] The instrument of Patent Document 1 takes a form in which it is possible to individually push out a plurality of suture instruments housed in the internal portion of the puncture needle (hereinafter referred to as repeating type). Specifically, four suture instruments are housed in the puncture needle. Thus, upon fixing the gastric wall to the body wall at three to four positions in one procedure, it is possible to achieve this with one instrument. Therefore, it is easy because the replacement operation such as discarding, opening or switching of the instrument becomes unnecessary.

    [0007] In the suturing tool of Patent Document 1, one suture is engaged with the engaging portion. When fixing the suturing tool to the body surface, the suture is tied together with the suture of the other suture instrument on the body surface.

    [0008] Similarly, in Patent Document 2, an organ fixing instrument using a T-shaped suture instrument is described. This instrument uses a suture instrument in which one looped suture is engaged with the rod-shaped engaging portion (rod). This organ fixing instrument takes a form in which only one suturing tool is equipped with in the internal site of the puncture needle (hereinafter referred to as single-type). The pad-like cushioning material is attached to the suture. The cushioning material is pressed against to the body surface, and the sutures entangled with each other are tied to the cushion material in a state of the two strings by cutting the suture loop. Thus, it is possible to fix each individual suture instrument to the body surface at 3 to 4 points of the fixed locations.

    DISCLOSURE OF THE INVENTION


    Problems to be Solved by the Invention



    [0014]  The organ fixing instrument of Patent Document 1 has a problem in which the sutures easily entangled each other because all of four sutures are inserted into the interior of the puncture needle. When the sutures tangle, there is a possibility in which the other locking part is also withdrawn when attempted to withdraw the puncture needle from the body wall by extruding the engaging portion of the tip from the puncture needle.

    [0015] Furthermore, the organ fixing instrument of Patent Document 1 has a problem in which the puncture needle is thick and highly invasive. That is to say, in the organ fixing instrument of Patent Document 1, four sutures are dispersedly arranged around the top, that is the locking portion which goes through the inside of the puncture needle of the extrusion apparatus side when accommodating the four suture instruments in the interior of the puncture needle. Therefore, the inner diameter of the puncture needle is required in the dimension of at least, in addition to the outer diameter of the engaging portion, the sum of the thickness of the two sutures on the both sides. Thus, the outer diameter of the puncture needle of the organ fixing instrument of Patent Document 1 is thick compared with that of the single type which contains only one suturing device.

    [0016] Accordingly, in the instrument of Patent Document 1, there is a risk of difficult operation due to tangled sutures, and there is a problem of highly invasive due to thick puncture needle, although there is a benefit of the procedure caused by the repeating type.

    [0017] Furthermore, in the organ fixing instrument of Patent Document 2, one instrument is required for each fixing point between the stomach wall and the body wall since it is a single type. The replacement of the instrument is complicated, and there is a problem of high cost.

    [0018] The present invention has been made in view of the above problems. The present invention has the benefits of repeated type in which the replacement works such as discarding, opening or switching of the instrument are reduced and simple. In addition, the present invention provides the organ fixing instrument capable of improving the stability of operation by suppressing the entanglement of the sutures than the conventional repeating type organ fixing instrument and realizing low invasion caused by thinning the puncture needle.

    Means for Solving the Problems



    [0019]  According to the first aspect of the present invention, a repeating type organ fixing instrument which has a plurality of suture instruments comprising a rod-shaped engaging portion and a suture which is connected to the engaging portion, and a puncture needle accommodating a plurality of the engaging portions, in which the plurality of engaging portions are extruded from the puncture needle one by one by the operation of the operating main unit, is provided, wherein the suture connected to at least one of the plurality of engaging portions is inserted into the inside of the puncture needle, and the other suture is provided to the outside from the inside of the puncture needle.

    [0020] According to the second aspect of the present invention, a repeating type organ fixing instrument which has a suture instrument comprising a rod-shaped engaging portion and a plurality of sutures which are connected to the engaging portion at their ends, and a puncture needle accommodating a plurality of the engaging portions side by side in the tip base end direction, in which the plurality of engaging portions are extruded from the puncture needle one by one by the operation of the operating main unit, is provided, wherein the yarn passage for inserting the suture housed in the tip end side of the suturing tool in the puncture needle is formed at the engaging portion of the other suturing tool housed in the base end.

    Effects of the invention



    [0021] According to the first aspect of the present invention, it is possible to reduce the number of the sutures going through the interior of the puncture needle. Therefore, a repeating type organ fixing instrument in which the stability of the operation due to a decrease in entanglement of the sutures in the interior of the puncture needle is improved, is provided, wherein it is possible to prevent expansion of the outer diameter of the puncture needle.

    [0022] According to the second aspect of the present invention, it is possible to insert a suture into the interior of the puncture needle through the yarn passage formed at the engaging portion. Thus, while having the advantages of the repeated type, low invasive organ fixing instrument can be realized because it is possible to reduce the diameter of the puncture needle.

    [0045] Therefore, according to the organ fixing instrument 100 of the present embodiment, it is possible to thin the outer diameter of the puncture needle 30 in comparison with the instrument of Patent Document 1, in which a plurality of sutures are passed on both sides of the periphery of the engaging portion by facing. For convenience, a state in which the engaging portions 21, 21a and the sutures 22, 22a are loosely inserted into the puncture needle 30 is shown in FIG.12. However, as described above, the inner diameter of the puncture needle 30 is preferably the first total length or more and less than the second total length, and more preferably the first total length or more and less than the third total length.

    [0046] The penetrating portion, that is to say, the slit 33 penetrating the inside and outside, through which the suture 22 of the suturing tool 20 of the first stage passes, is provided at the puncture needle 30, as shown in FIG. 1. The penetrating portion is a portion which becomes the path for inserting the suture 22 to the inside and outside of the puncture needle 30, and is formed by penetrating the inside and outside of the lateral of the puncture needle 30 in the axial direction. The penetrating portion may be provided with the slit 33 as the present embodiment, it may comprise a transverse hole (see the third embodiment), and may comprise both.

    [0047] As shown in FIG. 1, the slit 33 extends toward the operation unit main body 63 from the needle tip 31 of the puncture needle 30. In the slit 33, one end portion is open to the needle tip 31, the other end portion is linearly extending toward the base end side of the puncture needle 30, that is to say, toward the extrusion apparatus 40 along the axial direction of the puncture needle 30, and terminates in the intermediate portion of the puncture needle 30 (the tip end side from the base end side of the puncture needle 30) to form a dead end. However, it may extend to the extrusion apparatus 40 side (base end side), longer than this. The groove width of the slit 33 is set to the thickness of one suture 22 or more.

    [0048] In the present embodiment, since there are two suturing tools 20, 20a, as shown in FIG. 12, one of the sutures 22, 22a, that is to say, the suture 22 of the first stage passes the outside of the puncture needle 30, other one, that is to say, the suture 22a of the second stage of goes through the interior.

The cylindrical portion 50 is open at both ends, the base end portion of the puncture needle 30 which is the opposite side of the needle tip 31 is fixed to one opening end portion, the inside of the cylindrical portion 50 communicates with the housing portion 32 of the puncture needle 30 through this one end portion, and the operation unit 60 is inserted into the opening of the other end.

The suture 22a derived to inside of the cylindrical portion 50 through the housing portion 32 of the puncture needle 30 goes through the thread exit hole 51.

    [0055] The operation unit 60 extrudes the engaging portions 21, 21a housed in the housing portion 32 of the puncture needle 30 from the needle tip 31 of the puncture needle 30 through the interior of the cylindrical portion 50.

    [0056] The operation unit 60 comprises a push rod 61, a pressing portion 62, and an operation unit main body 63, as shown in FIGS. 2 to 7, when roughly divided.

    [0057] The push rod 61 is a stainless steel mandrel, as shown in FIGS. 4 and 8, and extrudes the engaging portions 21, 21a housed in the housing portion 32 from the needle tip 31 by being inserted into the housing portion 32 of the puncture needle 30 from the interior of the cylindrical portion 50, and sliding in the housing portion 32. That is to say, since the first stage of the engaging portion 21 and the second stage of the engaging portion 21a positioned at the next stage are accommodated in this order from the needle tip 31, the push rod 61 comes into contact with the second stage of the engaging portion 21a. When the push rod 61 proceeds to the needle tip 31 side (tip end side of the puncture needle 30), the second stage of engaging portion 21a proceeds to the needle tip 31 side by being pushed by the push rod 61. Therefore, the first stage of the engaging portion 21 also proceeds to the needle tip 31 side by being pushed by the second stage of the engaging portion 21a.

    Claims

    1. A repeated type organ fixing instrument (100) having a plurality of suturing tools (20, 20a to 20c) comprising a rod-shaped engaging portion (21, 21a to 21c), a suture (22, 22a to 22c) fixed to the engaging portion (21, 21a to 21c), a puncture needle (30) for housing a plurality of the engaging portions (21, 21a to 21c), wherein the plurality of the engaging portions (21, 21a to 21c) are extruded one by one from the puncture needle (30) by the operation of a operation unit main body (63), wherein the suture (22, 22a to 22c) fixed to at least one of the plurality of engaging portions (21, 21a to 21c) is inserted within the inside of the puncture needle (30),
    characterized in that
    the other suture (22, 22a to 22c) is derived from the inside to the outside of the puncture needle (30).
     
    2. The repeated type organ fixing instrument (100) according to claim 1, wherein a penetrating portion (33) which penetrates inside and outside of the puncture needle (30), and through which the other suture (22, 22a to 22c) passes is provided.
     
    3. The repeated type organ fixing instrument (100) according to claim 2, wherein the penetrating portion (33) has a slit extending toward the operation unit main body (63) from a needle tip (31) which is the tip of the puncture needle (30).
     
    4. The repeated type organ fixing instrument (100) according to any one of claims 1 to 3, wherein the instrument has two suturing tools (20, 20a to 20c), one suture (22, 22a to 22c) passes outside of the puncture needle (30), and the other one suture (22, 22a to 22c) passes through inside of the puncture needle (30).
     
    5. The repeated type organ fixing instrument (100) according to claim 4, wherein an inner diameter of the puncture needle (30) is a first total length or more, in which a minor axis of the engaging portion (21, 21a to 21c) is added with a minor axis of the suture (22, 22a to 22c), and less than a second total length in which a major axis of the engaging portion (21, 21a to 21c) is added with major axes of the two sutures (22, 22a to 22c).
     
    6. The repeated type organ fixing instrument (100) according to claim 3, wherein the instrument has three or more suturing tools (20, 20a to 20c), the slit (200; 400) extends from an arrangement region of the engaging portion (21, 21a to 21c) located at the tip end side of the puncture needle (30) to at least a middle of an arrangement region of the engaging portion (21, 21a to 21c) located at a next stage of the base end side of the puncture needle (30), and wherein the suture (22, 22a to 22c) of the engaging portion (21, 21a to 21c) located at the next stage is inserted into the slit (200; 400).
     
    7. The repeated type organ fixing instrument (100) according to claim 6, wherein the slit (400) is formed in a spiral shape.
     
    8. The repeated type organ fixing instrument (100) according to any one of claims 1 to 3, 6 and 7, wherein the instrument has three or more suturing tools (20, 20a to 20c), the sutures (22, 22a to 22c) of the two suturing tools (20, 20a to 20c) among three or more sutures (22, 22a to 22c), are derived from the inside to the outside of the puncture needle (30), and wherein the sutures (22, 22a to 22c) are opposed with respect to the puncture needle (30).
     
    9. The repeated type organ fixing instrument (100) according to any one of claims 2, 3, 6, 7 and claim 8 which is according to claim 2, wherein the instrument has a transverse hole (300) penetrating inside and outside of the puncture needle (30) as the penetrating portion (33).
     
    10. The repeated type organ fixing instrument (100) according to claim 9, wherein an end face of a tip end side of the transverse hole (300) is inclined from a tip end direction to a base end direction as the end face goes from the inside to the outside.
     
    11. A repeated type organ fixing instrument (100') having a plurality of suturing tools (20', 20a' to 20c') comprising a rod-shaped engaging portion (21', 21a' to 21c'), a suture (22', 22a' to 22c') in which an end thereof is fixed to the engaging portion (22', 22a' to 22c'), and a puncture needle (30') for housing a plurality of the engaging portions (22', 22a' to 22c') side by side in a tip base direction, wherein the plurality of the engaging portions (22', 22a' to 22c') are extruded one by one from the puncture needle (30') by the operation of a operation unit main body (63'),
    characterized in that
    a string passage (23a' to 23d') for inserting the suture (22', 22a' to 22c') of the suturing tool (20', 20a' to 20c') housed at a tip end side into an inside of the puncture needle (30') is formed at the engaging portion (22', 22a' to 22c') of the other suturing tool (20', 20a' to 20c') housed at a base end side.
     
    12. The repeated type organ fixing instrument (100') according to claim 11, wherein the engaging portion (22', 22a' to 22c') is formed to be hollow and an inside of a hollow cylinder of the engaging portion (22', 22a' to 22c') is the string passage (23a' to 23d').
     
    13. The repeated type organ fixing instrument (100') according to claim 11 or 12, wherein the string passage (23a' to 23d') is formed in a groove shape at the peripheral surface of the engaging portion (22', 22a' to 22c') along an extrusion direction of the engaging portion (22', 22a' to 22c').

    19. The repeated type organ fixing instrument (100') according to any one of claims 11 to 18, wherein the engaging portion (22', 22a' to 22c') is formed to be hollow, an through hole (310') penetrating inside and outside of the engaging portion (22', 22a' to 22c') is formed at the middle in the longitudinal direction, wherein the suture (22', 22a' to 22c') inserted from the through hole (310') is fixed to the inside of the hollow by swaging around the engaging portion (22', 22a' to 22c'), and wherein the suture (22', 22a' to 22c') derived from the through hole (310') to the outside is inserted into the string passage (23a' to 23d').
     


    Ansprüche

    1. Ein Organfixierinstrument vom Wiederholungstyp (100) mit einer Vielzahl von Nahtwerkzeugen (20, 20a bis 20c), umfassend einen stabförmigen Eingriffsabschnitt (21, 21a bis 21c), ein Nahtmaterial (22, 22a bis 22c), das am Eingriffsabschnitt (21, 21a bis 21c) befestigt ist, eine Punktionsnadel (30) zur Aufnahme einer Vielzahl der Eingriffsabschnitte (21, 21a bis 21c), wobei die Vielzahl der Eingriffsabschnitte (21, 21a bis 21c) nacheinander Stück für Stück von der Punktionsnadel (30) durch den Betrieb eines Operationseinheit-Hauptkörpers (63) ausgestoßen werden (extruded), wobei das Nahtmaterial (22, 22a bis 22c), das an mindestens einem der Vielzahl von Eingriffsabschnitten (21, 21a bis 21c) befestigt ist, in das Innere der Punktionsnadel (30) eingesetzt wird,
    dadurch gekennzeichnet, dass
    das weitere Nahtmaterial (22, 22a bis 22c) von der Innenseite zur Außenseite der Punktionsnadel (30) abgeleitet wird (derived from).
     
    2. Das Organfixierinstrument vom Wiederholungstyp (100) nach Anspruch 1, wobei ein durchdringender Abschnitt (33) bereitgestellt wird, der in die und aus der Punktionsnadel (30) dringt und durch den das weitere Nahtmaterial (22, 22a bis 22c) hindurchgeht.
     
    3. Das Organfixierinstrument vom Wiederholungstyp (100) nach Anspruch 2, wobei der durchdringende Abschnitt (33) einen Schlitz aufweist, der sich von einer Nadelspitze (31), die die Spitze der Punktionsnadel (30) ist, in Richtung des Operationseinheit-Hauptkörpers (63) erstreckt.
     
    4. Das Organfixierinstrument vom Wiederholungstyp (100) nach irgendeinem der Ansprüche von 1 bis 3, wobei das Instrument zwei Nahtwerkzeuge (20, 20a bis 20c) aufweist, wobei ein Nahtmaterial (22, 22a bis 22c) außerhalb der Punktionsnadel (30) verläuft und das weitere Nahtmaterial (22, 22a bis 22c) innerhalb der Punktionsnadel (30) verläuft.
     
    5. Das Organfixierinstrument vom Wiederholungstyp (100) nach Anspruch 4, wobei ein Innendurchmesser der Punktionsnadel (30) eine erste Gesamtlänge oder mehr aufweist, bei der eine Nebenachse des Eingriffsabschnitts (21, 21a bis 21c) mit einer Nebenachse des Nahtmaterials (22, 22a bis 22c) ergänzt wird, und weniger als eine zweite Gesamtlänge aufweist, bei der eine Hauptachse des Eingriffsabschnitts (21, 21a bis 21c) mit Hauptachsen der beiden Nahtmaterialien (22, 22a bis 22c) ergänzt wird.
     
    6. Das Organfixierinstrument vom Wiederholungstyp (100) nach Anspruch 3, wobei das Instrument drei oder mehr Nahtwerkzeuge (20, 20a bis 20c) aufweist, wobei sich der Schlitz (200; 400) von einem Anordnungsbereich des Eingriffsabschnitts (21, 21a bis 21c), der sich an der Spitzenendseite der Punktionsnadel (30) befindet, zumindest bis zu einer Mitte eines Anordnungsbereichs des Eingriffsabschnitts (21, 21a bis 21c) erstreckt, die sich in einer nächsten Stufe der Basisendseite der Punktionsnadel (30) befindet, und wobei das Nahtmaterial (22, 22a bis 22c) des in der nächsten Stufe befindlichen Eingriffsabschnitts (21, 21a bis 21c) in den Schlitz (200; 400) eingeführt wird.
     
    7. Das Organfixierinstrument vom Wiederholungstyp (100) nach Anspruch 6, wobei der Schlitz (400) spiralförmig ausgebildet ist.
     
    8. Das Organfixierinstrument vom Wiederholungstyp (100) nach irgendeinem der Ansprüche von 1 bis 3, 6 und 7, wobei das Instrument drei oder mehr Nahtwerkzeuge (20, 20a bis 20c) aufweist, wobei die Nahtmaterialien (22, 22a bis 22c) der zwei Nahtwerkzeuge (20, 20a bis 20c) unter den drei oder mehr Nahtmaterialien (22, 22a bis 22c) abgeleitet werden von der Innenseite zur Außenseite der Punktionsnadel (30), und wobei die Nahtmaterialien (22, 22a bis 22c) bezüglich der Punktionsnadel (30) gegenüberliegen.
     
    9. Das Organfixierinstrument vom Wiederholungstyp (100) nach irgendeinem der Ansprüche 2, 3, 6, 7 und Anspruch 8, entsprechend Anspruch 2, wobei das Instrument ein Querloch (300) aufweist, das als durchdringender Abschnitt (33) in die und aus der Punktionsnadel (30) dringt.
     
    10. Das Organfixierinstrument vom Wiederholungstyp (100) nach Anspruch 9, wobei eine Endfläche einer Spitzenendseite des Querlochs (300) von einer Spitzenendrichtung zu einer Basisendrichtung geneigt ist, wenn die Endfläche von innen nach außen verläuft.
     
    11. Ein Organfixierinstrument vom Wiederholungstyp (100') mit einer Vielzahl von Nahtwerkzeugen (20', 20a' bis 20c'), umfassend einen stabförmigen Eingriffsabschnitt (21', 21a' bis 21c'), ein Nahtmaterial (22', 22a' bis 22c'), in dem ein Ende davon am Eingriffsabschnitt (22', 22a' bis 22c') befestigt ist, und eine Punktionsnadel (30') zur Aufnahme einer Vielzahl der Eingriffsabschnitte (22', 22a' bis 22c') Seite an Seite in einer Spitzenbasisrichtung, wobei die Vielzahl der Eingriffsabschnitte (22', 22a' bis 22c') nacheinander Stück für Stück von der Punktionsnadel (30') durch den Betrieb eines Operationseinheit-Hauptkörpers (63') ausgestoßen werden (extruded),
    dadurch gekennzeichnet, dass
    ein Strangdurchgang bzw. Schnurgang (string passage) (23a' bis 23d') zum Einführen des Nahtmaterials (22', 22a' bis 22c') des Nahtwerkzeugs (20', 20a' bis 20c'), das an einer Spitzenendseite aufgenommen ist, in eine Innenseite der Punktionsnadel (30'), am Eingriffsabschnitt (22', 22a' bis 22c') des weiteren Nahtwerkzeugs (20', 20a' bis 20c') ausgebildet ist, der an einer Basisendseite aufgenommen ist.
     
    12. Das Organfixierinstrument vom Wiederholungstyp (100') nach Anspruch 11, wobei der Eingriffsabschnitt (22', 22a' bis 22c') ausgebildet ist, um hohl zu sein, und eine Innenseite eines Hohlzylinders des Eingriffsabschnitts (22', 22a' bis 22c') der Strangdurchgang (23a' bis 23d') ist.
     
    13. Das Organfixierinstrument vom Wiederholungstyp (100') nach Anspruch 11 oder 12, wobei der Strangdurchgang (23a' bis 23d') in Nutform an der Umfangsfläche des Eingriffsabschnitts (22', 22a' bis 22c') entlang einer Ausstoßrichtung des Eingriffsabschnitts (22', 22a' bis 22c') ausgebildet ist.
     
    14. Das Organfixierinstrument vom Wiederholungstyp (100') nach irgendeinem der Ansprüche von 11 bis 13, wobei eine Vielzahl der Strangdurchgänge (23a' bis 23d') gebildet sind und eine Vielzahl der Strangdurchgänge (23a' bis 23d') gebildet und verteilt sind (dispersed), um voneinander getrennt zu sein.
     
    15. Das Organfixierinstrument vom Wiederholungstyp (100') nach Anspruch 14, wobei drei oder mehr Strangdurchgänge (23a' bis 23d') gebildet sind und eine Vielzahl der Strangdurchgänge (23a' bis 23d') gleichmäßig gebildet und in Umfangsrichtung auf der Oberfläche des Eingriffsabschnitts (22', 22a' bis 22c') verteilt sind.
     
    16. Das Organfixierinstrument vom Wiederholungstyp (100') nach irgendeinem der Ansprüche von 11 bis 15, wobei um den in Längsrichtung mittleren Abschnitt des Eingriffsabschnitts (22', 22a' bis 22c') eine ringförmige Fixiernut (200') zur Fixierung durch Binden des Nahtmaterials (22', 22a' bis 22c') gebildet wird.
     
    17. Das Organfixierinstrument vom Wiederholungstyp (100') nach irgendeinem der Ansprüche von 11 bis 16, wobei ein unebener Abschnitt (600', 610') an der Endfläche des Eingriffsabschnitts (22', 22a' bis 22c') gebildet wird und der unebene Abschnitt (600') und der unebene Abschnitt (610') des weiteren benachbarten Eingriffsabschnitts (22', 22a' bis 22c') ineinander eingepasst werden.
     
    18. Das Organfixierinstrument vom Wiederholungstyp (100') nach Anspruch 17, wobei der unebene Abschnitt (600', 610') so ausgebildet ist, dass ein Querschnitt unrund ist.
     
    19. Das Organfixierinstrument vom Wiederholungstyp (100') nach irgendeinem der Ansprüche von 11 bis 18, wobei der Eingriffsabschnitt (22', 22a' bis 22c') ausgebildet ist, um hohl zu sein, wobei ein Durchgangsloch (310'), das innerhalb und außerhalb des Eingriffsabschnitts (22', 22a' bis 22c') dringt, in der Mitte in Längsrichtung ausgebildet ist, wobei das aus dem Durchgangsloch (310') eingeführte Nahtmaterial (22', 22a' bis 22c') an der Innenseite des Hohlraums befestigt ist, indem es um den Eingriffsabschnitt (22', 22a' bis 22c') herum geknickt bzw. gesenkgeschmiedet wird (swaging), und wobei das aus dem Durchgangsloch (310') nach außen abgeleitete Nahtmaterial (22', 22a' bis 22c') in den Strangdurchgang (23a' bis 23d') eingeführt wird.
     


    Revendications

    1. Un instrument de fixation d'organe de type répété (100) présentant une pluralité d'outils de suture (20, 20a à 20c) comprenant une partie d'engagement en forme de tige (21, 21a à 21c), une suture (22, 22a à 22c) fixée à la partie d'engagement (21, 21a à 21c), une aiguille à ponction (30) pour loger une pluralité des parties d'engagement (21, 21a à 21c), sachant que la pluralité des parties d'engagement (21, 21a à 21c) sont extrudées une à une de l'aiguille de ponction (30) par l'actionnement d'un corps principal d'unité d'opération (63), sachant que la suture (22, 22a à 22c), fixée à au moins une partie parmi la pluralité des parties d'engagement (21, 21a à 21c), est introduite à l'intérieur de l'aiguille de ponction (30),
    caractérisé en ce que
    l'autre suture (22, 22a à 22c) est dérivée de l'intérieur vers l'extérieur de l'aiguille de ponction (30).
     
    2. L'instrument de fixation d'organe de type répété (100) d'après la revendication 1, sachant qu'on fournit une partie pénétrante (33) qui pénètre à l'intérieur et à l'extérieur de l'aiguille de ponction (30) et par laquelle passe l'autre suture (22, 22a à 22c).
     
    3. L'instrument de fixation d'organe de type répété (100) d'après la revendication 2, sachant que la partie pénétrante (33) présente une fente s'étendant vers le corps principal d'unité de commande (63) depuis une pointe d'aiguille (31) qui est la pointe de l'aiguille de ponction (30).
     
    4. L'instrument de fixation d'organe de type répété (100) d'après l'une quelconque des revendications de 1 à 3, sachant que l'instrument présente deux outils de suture (20, 20a à 20c), une suture (22, 22a à 22c) passant à l'extérieur de l'aiguille de ponction (30), et l'autre suture (22, 22a à 22c) passant à l'intérieur de l'aiguille de ponction (30).
     
    5. L'instrument de fixation d'organe de type répété (100) d'après la revendication 4, sachant qu'un diamètre intérieur de l'aiguille de ponction (30) est d'une première longueur totale ou plus, dans laquelle un axe mineur de la partie d'engagement (21, 21a à 21c) est ajouté à un axe mineur de la suture (22, 22a à 22c), et de moins d'une deuxième longueur totale, dans laquelle un axe majeur de la partie d'engagement (21, 21a à 21c) est ajouté à des axes majeurs des deux sutures (22, 22a à 22c).
     
    6. L'instrument de fixation d'organe de type répété (100) d'après la revendication 3, sachant que l'instrument présente trois outils de suture ou plus (20, 20a à 20c), que la fente (200 ; 400) s'étend depuis une région d'agencement de la partie d'engagement (21, 21a à 21c) située du côté de l'extrémité frontale de l'aiguille de ponction (30) vers au moins une partie centrale d'une région d'agencement de la partie d'engagement (21, 21a à 21c) située au stade suivant du côté d'extrémité de base de l'aiguille de ponction (30), et sachant que la suture (22, 22a à 22c) de la partie d'engagement (21, 21a à 21c), située à l'étape suivante, est introduite dans la fente (200 ; 400).
     
    7. L'instrument de fixation d'organe de type répété (100) d'après la revendication 6, sachant que la fente (400) est formée en forme de spirale.
     
    8. L'instrument de fixation d'organe de type répété (100) d'après l'une quelconque des revendications de 1 à 3, 6 et 7, sachant que l'instrument présente trois outils de suture ou plus (20, 20a à 20c), que les sutures (22, 22a à 22c) des deux outils de suture (20, 20a à 20c) parmi trois sutures ou plus (22, 22a à 22c), sont dérivées depuis l'intérieur vers l'extérieur de l'aiguille de ponction (30), et sachant que les sutures (22, 22a à 22c) sont opposées par rapport à l'aiguille de ponction (30).
     
    9. L'instrument de fixation d'organe de type répété (100) d'après l'une quelconque des revendications 2, 3, 6, 7 et de la revendication 8 qui se conforme à la revendication 2, sachant que l'instrument présente un trou transversal (300) pénétrant à l'intérieur et à l'extérieur de l'aiguille de ponction (30) comme partie de pénétration (33).
     
    10. L'instrument de fixation d'organe de type répété (100) d'après la revendication 9, sachant qu'une face d'extrémité d'un côté d'extrémité de pointe du trou transversal (300) est inclinée depuis une direction d'extrémité de pointe vers une direction d'extrémité de base lorsque la face d'extrémité va de l'intérieur vers l'extérieur.
     
    11. Un instrument de fixation d'organe de type répété (100') présentant une pluralité d'outils de suture (20', 20a' à 20c') comprenant une partie d'engagement en forme de tige (21', 21a' à 21c'), une suture (22', 22a' à 22c') dont une extrémité est fixée à la partie d'engagement (22', 22a' à 22c') et une aiguille de ponction (30') pour loger une pluralité des parties d'engagement (22', 22a' à 22c') côte à côte dans une direction base pointe, sachant que la pluralité des parties d'engagement (22', 22a' à 22c') sont extrudées une à une depuis l'aiguille de ponction (30') par l'actionnement d'un corps principal d'unité de commande (63'),
    caractérisé en ce que
    un passage de cordon (23a' à 23d'), pour l'insertion de la suture (22', 22a' à 22c') de l'outil de suture (20', 20a' à 20c') logée sur un côté d'extrémité de pointe dans une face intérieure de l'aiguille de ponction (30'), est formé au niveau de la partie d'engagement (22', 22a' à 22c') de l'autre outil de suture (20', 20a' à 20c') logée sur le côté d'extrémité de base.
     
    12. L'instrument de fixation d'organe de type répété (100') d'après la revendication 11, sachant que la partie d'engagement (22', 22a' à 22c') est formée pour être creuse et que l'intérieur d'un cylindre creux de la partie d'engagement (22', 22a' à 22c') est le passage de cordon (23a' à 23d').
     
    13. L'instrument de fixation d'organe de type répété (100') d'après la revendication 11 ou 12, sachant que le passage de cordon (23a' à 23d') est formé en forme de rainure à la surface périphérique de la partie d'engagement (22', 22a' à 22c') le long d'une direction d'extrusion de la partie d'engagement (22', 22a' à 22c').
     
    14. L'instrument de fixation d'organe de type répété (100') d'après l'une quelconque des revendications de 11 à 13, sachant qu'une pluralité des passages de cordons (23a' à 23d') sont formés et qu'une pluralité des passages de cordons (23a' à 23d') sont formés et dispersés de manière à être séparés les uns des autres.
     
    15. L'instrument de fixation d'organe de type répété (100') d'après la revendication 14, sachant que trois passages de cordons ou plus (23a' à 23d') sont formés et qu'une pluralité des passages de cordons (23a' à 23d') sont formés et dispersés de manière uniforme dans la direction circonférentielle à la surface de la partie d'engagement (22', 22a' à 22c').
     
    16. L'instrument de fixation d'organe de type répété (100') d'après l'une quelconque des revendications de 11 à 15, sachant qu'une rainure de fixation annulaire (200') destinée à être fixée en attachant la suture (22', 22a' à 22c') est formée autour de la partie centrale dans la direction longitudinale de la partie d'engagement (22', 22a' à 22c').
     
    17. L'instrument de fixation d'organe de type répété (100') d'après l'une quelconque des revendications de 11 à 16, sachant qu'une partie inégale (600', 610') est formée à la surface d'extrémité de la partie d'engagement (22', 22a' à 22c') et que la partie inégale (600') et la partie inégale (610') d'une autre partie d'engagement adjacente (22', 22a' à 22c') sont ajustées (fitted into) l'une dans l'autre.
     
    18. L'instrument de fixation d'organe de type répété (100') d'après la revendication 17, sachant que la partie inégale (600', 610') est formée de manière qu'une section transversale soit non-circulaire.
     
    19. L'instrument de fixation d'organe de type répété (100') d'après l'une quelconque des revendications de 11 à 18, sachant que la partie d'engagement (22', 22a' à 22c') est formée pour être creuse, qu'un trou traversant (310') pénétrant l'intérieur et l'extérieur de la partie d'engagement (22', 22a' à 22c') est formé au milieu dans le sens longitudinal, sachant que la suture (22', 22a' à 22c') insérée depuis le trou traversant (310') est fixée à l'intérieur de la cavité en sertissant (swaging) autour de la partie d'engagement (22', 22a' à 22c'), et sachant que la suture (22', 22a' à 22c') derivée depuis le trou traversant (310') vers l'extérieur est introduite dans le passage de cordon (23a' à 23d').
